چکیده

Michael C. Rowbotham, MD California Pacific Medical Center Research Institute Department of Anesthesia, UCSF School of Medicine San Francisco, Calif., USA Email: [email protected] P ostherpetic neuralgia (PHN) is t e most frequent chronic complication of herpes zoster ( hingles). Herpes zoster (HZ) represents a reactivation of the varicella zo ter vi us (VZV), a ubiquitous, highly neurotropic, exclusively human α-herpesvirus. rimary infection causes varicella (chickenpox), after which VZV becomes latent in sensory ganglia along the entire neuraxis. With the decline in VZV-specific cell-mediated immunity in elderly and immunocompromised individuals, VZV reactivates to cause HZ, characterized by a painful maculopapular or vesicular rash in all or part of the skin territory innervated by a single dorsal root ganglion. The most common site of HZ is thoracic, comprising about half of all cases, followed by trigeminal (usually the ophthalmic branch), cervical, and lumbar distributions. HZ generally resolves within a few weeks, but a minority of patients experience pain (PHN) persisting for months, years, or even a lifetime.

Herpes zoster: medical and nursing management.

Herpes zoster, the latent descendent of the varicella zoster virus, commonly is seen in clinical practice. Healthcare providers must recognize and treat the virus to decrease the incidence of postherpetic neuralgic pain syndrome. Treatment with an antiviral medication regimen should be initiated rapidly for patients who have had lesions for up to 72 hours. Acyclovir has been the treatment of ch...

Treatment of postherpetic neuralgia: focus on pregabalin

Postherpetic neuralgia (PHN) is a devastating, chronic pain syndrome that can develop following an outbreak of herpes zoster and becomes increasingly common as patients age. PHN can be difficult to treat and often requires trials of multiple agents to achieve significant pain relief. Pregabalin is the newest agent to gain approval for PHN. Data suggest efficacy for relief of pain and sleep dist...

Recurrent herpes zoster with segmental paresis and postherpetic neuralgia.

INTRODUCTION Postherpetic neuralgia and segmental paresis represent rare complications of herpes zoster infection. Recurrent herpes zoster is also rare and occurs within the first 3 years of the beginning of the illness in only 1.40% of cases but it is generally higher in cases of chronic lymphatic leukemia (3.5%). CASE REPORT We presented a patient with lymphatic leukemia who during the remi...

The burden of herpes zoster and postherpetic neuralgia in the United States.

Approximately one in three persons will develop herpes zoster during their lifetime, and it can lead to serious complications such as postherpetic neuralgia. However, evidence on burden of herpes zoster and postherpetic neuralgia in Japan is limited. This prospective, observational, multicenter, physician practice-based cohort study was conducted in Kushiro, Hokkaido, Japan (Clinicaltrials.gov ...
